Objective In Iran, the family physician program has been implemented as a pilot since 2008. There are challenges in the implementation of this plan in Minab City, Hormozgan Province, south of Iran. The present study aims to investigate these challenges.
Methods This qualitative study was conducted during 2022-2023. The participants were 10 experts (the family physician program experts, the head of the health and treatment network, physicians, and health care workers in Minab City). The sampling was done using a purposive sampling method until reaching the saturation stage. Semi-structured interviews and field notes were used to collect information. Data analysis was done using thematic content analysis method. 
Results We extracted five main themes and 10 categories including human resources (midwives/health care workers, physicians/specialists, managers), credits (salary payment, amenities), rules (physicians/specialists, patients, midwives/health care workers), system security (staff), and standard policy (health system policy), as well as 20 sub-categories. 
Conclusion The family physician program in Minab City has faced challenges due to reasons such as low budget, improper management, and insufficient justification of the staff. It is recommended to solve these challenges so that the residents of this city can benefit from this plan.
